If you already have a hearing scheduled, this form must be received at least three (3) business days prior to the time of the scheduled hearing. This form must be filled out completely. The court will send the Judge's decision by mail. Therefore, you must provide a current and valid address. Failure to pay an assessed penalty may result in Suspension of your Driving Privilege (Suspended Driver's License), additional fees and/or referral to a collection agency.
IMPORTANT NOTICE:
As a user of the Electronic Mitigation of Infraction Program, it is your responsibility to contact the Sunnyside Municipal Court within fourteen (14) days at (509) 839-4427 or (509) 836-6278 if you have not received a response from the Court. The City does not accept liability for any damage or injury caused by the use of this program.
